Terms of Business – Podiatry StationBy accepting our services or purchase of goods, you the client contractually agree to the Terms of Business as laid out below. These terms may be amended and/or updated without any prior notice. Patient RecordsPatient records including any photographs and similar documents are the property of, and will be retained by Podiatry Station in the interest of patient welfare. At the patients’ request, we will provide copies of any relevant clinical records. There is a £20 charge for this service. FeesAll fees are subject to VAT at the current rate. Fee levels are determined by the services provided and according to the resources, materials and consumables used. Treatment Plans & CostsIf required, we will happily provide you a written estimate after an initial consultation and diagnosis as to the probable costs of a course of treatment. Please bear in mind that any estimate given can only be approximate. We will always try to keep you informed on costs as they arise. Payment Terms Invoices are to be settled immediately. We do not offer credit terms. New clients may be required to prepay or guarantee an appointment with a credit/debit card. We reserve the right to request payment prior to certain treatments and/or supply of goods. Payment MethodsIn Clinic - Cash or Debit/Credit Card payments are accepted. CancellationAll cancellations must be received a minimum of 48-hours prior to the appointment time. Cancellations made within 24-hours to 48-hours prior to the appointment time will be subject to a 50% charge of the standard appointment cost. Cancellations made within 24-hours prior to the appointment time will be subject to a 100% charge of the standard appointment cost. Missed AppointmentsFailure to attend an appointment is subject to a 100% charge of the standard appointment cost. Home VisitsPets must be kept locked away for the duration of an appointment; otherwise no treatment will be carried out. In this case, the appointment cost will still be due in full. Kindly ensure that the room in which treatment is to be carried within has enough space and is well lit. Please also refrain from smoking. ConductWe aim to run a professional, polite and friendly service at all times. Therefore we expect the same from patients and anyone else related to the patient who may be present throughout an appointment. We take abuse and rude behaviour very seriously. This will not be tolerated and no future appointment will be made available if any such situation occurs, especially where we may feel threatened. In some cases, we will not hesitate to report such behaviour to the Police for investigation. DisputesWe hope you never have reason to complain. However, if you feel that there is something you wish to complain about please direct your comments in writing within 3 working days to The Clinic Manager, Podiatry Station, 10 Station Road, Edgware, Midddlesex HA8 7AB. Any dispute regarding consultation, diagnostic or treatment must be raised immediately with the Podiatrist at the same time. |
